The Town of Allen is excited for the Oklahoma Health Departmant to bring their Public Health’s Mobile Clinic to Allen. They will bringing 100 vaccinations to this clinic.

The Public Health’s Mobile Clinic will be administering Johnson & Johnson COVID19 vaccine by appointment only. Where: School Gymnasium, 300 E Broadway, Allen, OK 74825

Time: 10:30am – 11:30am

To book your vaccine appointment you must be 18 years or older. There are two options to book an appointment.

Option 1: call 580-768-6100 Monday-Friday, from 8:30 am to 4:30pm.

Option 2: Book your appointment on line by following the below steps.

Step 1: go to vaccinate.oklahoma.gov

Step 2: scroll down and click on the “COVID19 Questionnaire” button. On the first page of the questionnaire it will ask “Did you receive an invitation code for a reserved vaccine site”? Answer YES and enter the following invitation code D6B9V5001172B4S1B8. At the end of questionnaire the registration portal will display all available appointment times for you to choose from. If you have already registered on line, use the email with the booking link sent to you and enter the above code.

On the day of your appointment please follow the below steps:

• Arrive at your designated appointment time, there is no need to arrive early.

• Your appointment time ensures you have a vaccine shot reserved for you.

• Eat before your appointment

• Wear a short sleeved shirt

• Bring a photo ID

• Please wear a mask
